Use http://www.vcardexport.com . Create a single vCard file. Delete old contacts in Lotus Notes, and then import vCard file. Individually open and then save each entry in Lotus Notes to create correct 1st/2nd name operation and correct order in view.

Lulu Cover Options, select: Dsiplay title on front cover and display author on front cover; Font & Colors, Font = Georgia; Colors, Font Color = #FFFFFF, Background/Spine Color = #0701FF; update cover if necessary and then save and continue
Lulu Pricing & Royalty, select Download and Print; leave prices unchanged; select Available only to me (published)
